Advanced Directive

An Advanced Directive allows you to choose someone to make health care decisions for you if you cannot make decisions, state your treatment preferences in the event you are unable to communicate, and addresses other specific situations regarding your health care. While completely optional, It is an important document that gives you a say in your care in case you are not able to express your wishes and allows family members and your health care team to know your wishes.  To download a copy of the Georgia Advance Directive for Healthcare, just click the Advance Directive button. If you choose to fill it out, be sure to give a copy to your health care agent, your physician, and keep a copy at home in a place that can be easily found. 
Advanced Directive Form
We recognize that not everyone has the resources they need to obtain medical care. For that reason, we are happy to offer Financial Assistance to those in need. If you have received HAMC services and find it would be a hardship to cover them financially, we encourage you to apply for financial assistance. If you reside in Miller, Baker or Calhoun County, we offer a Financial Assistance Program. For an appointment with a Financial Counselor to determine if you are eligible, please call (229) 758-3554. Your information will be evaluated for financial assistance.

If you do not reside in Miller, Baker or Calhoun County and you come to HAMC for non-emergency or non-specialty medical care, you will be required to pay in full for your medical services before you will be seen by a healthcare provider. We encourage all uninsured and underinsured patients to complete the financial assistance process so that you can receive the appropriate discount for services. Those who do not go through financial assistance will be asked to pay the full cost of their care.
